Spring Learning Series Examines Strategies for Improving Chronic Disease Management

The National Health Care for the Homeless Council invites health care professionals to join us at the Spring 2024 National Learning Series led by the Population Health Management Task Force (PHM), a coalition of 13 National Training and Technical Assistance Partner organizations, including the Council.

This series will take a deeper dive into issues around systemic, structural, and environmental barriers and strategies for improving chronic disease management and care.

Each webinar will focus on specific strategies for developing, evaluating, and supporting effective health care delivery models. Coordinators will ensure that webinar materials are practical, providing information and enhancing skills directly related to the training recipients.

Sessions will take place on Wednesdays in March.

  • March 6: Foundations of Population Health Management in Priority Populations
  • March 13: Housing and HIV: Bridging the Gap between HIV and Housing in Special and Vulnerable Populations
  • March 20: Diabetes Prevention through the lens of Food Sovereignty and Access to Ancestral, Cultural, and Healthful Foods
  • March 27: Team-Based Approaches for Managing Complex Health Needs
