Yakima Neighborhood Health Services
Yakima Neighborhood Health Services (YNHS), established in 1975, provides comprehensive medical, dental, behavioral health, and social service support to thousands of residents each year. In 2004, YNHS was a founding partner of the Homeless Network of Yakima County — a collective impact model of agencies to address the needs of individuals experiencing homelessness.
YNHS provides comprehensive medical, dental , behavioral health, vision, and basic needs assistance at nine locations in urban and rural settings in the Yakima Valley (Central Washington State). We are also a Coordinated Entry Access point for our local Continuum of Care, and provide permanent supportive housing and medical respite care for literally homeless individuals and families.
Services are provided by competent and culturally-appropriate health professionals and support staff. Pharmacy services are available. Patient Navigators help patients apply for health coverage as part of patient registration and intake.
A robust HCH outreach team (nurse, behavior health specialist, case manager, health educator, and housing specialist) provide targeted engagement services in the valley’s shelters, streets, and outdoor encampments. Outreach workers serve as a “human recall systems” , deployed to find patients needing follow-up care.
Homeless Prevention and Rapid Rehousing services are offered through our Homeless Resource Center.
