Swope Health Services
Swope Health Services’ Health Care for the Homeless Program, located in Swope Health Services, a community health center and behavioral health center, provides primary health care and support services – outreach, case management, outpatient, drug and alcohol services, counseling, medication, and dental for individuals who are homeless. U.S. Department of Housing and Urban Development (HUD) supportive service grants enable the program to provide prescription medication and collaboration with a behavioral health program to serve the chronic mentally ill.
Services
The HCH project is located within Swope Health Services and on-site primary care is provided through the following clinics: adult medicine, OB-GYN, pediatrics, dental, vision, and behavioral health, on-site auxiliary services include radiology, laboratory, pharmacy, and optical shop. Homeless outreach provides health and support services at shelters, soup kitchens, drop-in centers, treatment facilities, on the streets, homeless camps, and other sites where homeless persons congregate. 24 hour emergency on-call, Asthma, Basic Needs, Behavioral Health Care Services, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Chronic Bronchitis/Emphysema, Contraceptives (issued w/counseling), Dental Care, Diabetes Testing/Care, Family Planning, Flu Vaccinations, Gynecology, Health/Nutritional Education, Hepatitis C screening/treatment, HIV/AIDS screening/education, Immunizations, Labs, Medications/Prescription, Mental Health Counseling, Mental Health Services for Youth, Mobile Van/Transportation, Multidisciplinary Health Teams, Obstetrics, Optometry Services, Outreach, Pain Management Services, Pediatrics, Pharmacy, Physical Exams, Pneumococcal vaccine, Primary Health Care, Referrals for Specialty Care, Residential, Services in Supportive Housing, Smoking Cessation, Substance Abuse Treatment, Trauma, TB screening/education, Treatment of Acute Illness, Treatment of Chronic Illness, Urgent Care, Well Child Check-ups, X-ray
