RiverStone Health
RiverStone Health operates the only Healthcare for the Homeless (HCH) program in the eastern half of Montana. HCH patients are served at one of the RiverStone Health locations which include the main Community Health Center Clinic, a dedicated HCH clinic site and at a day shelter operated by a local nonprofit organization.
Services
Services include primary health care, preventative health care and screenings, Chronic disease management, labs, mental health care, substance abuse treatment, nursing care, dental services, outreach, case management and referrals to other needed community services. Testing and treatment options for STI’s, HIV, and Hepatitis C. All recommended immunizations are available as well as TB testing.
Same day care for children and adults of all ages, Pharmacy services are available, including assistance with prescription costs. RiverStone Health also manages the WIC nutrition program for Yellowstone, Carbon, Stillwater and Musselshell counties. HCH connects eligible patients to WIC.
Linkages
Linkages include community agencies which provide various services, including emergency shelter, meals, clothing, substance abuse treatment, respite care, residential services, a domestic violence shelter and vocational education.
