Description: By creating a family of cross-functional service providers to seamlessly support unhoused individuals, we begin to help provide the best opportunity for individuals to transition permanently from chronic homelessness. We also can begin to see massive gains in how we shift a highly utilizing population reliant upon episodic interventions to a more preventative care model. Learn how NHF’s intentionally designed staffing model provides a multi-disciplinary approach to tackling system challenges and sensitivities, purposefully operationalizing these sensitivities into actionable systems change.
