The Creating Care and Discharge Plans for People Experiencing Homelessness who are Hospitalized Toolkit was created by the National Health Care for the Homeless Council in partnership with CommonSpirit Health. This tool kit was developed thanks to the invaluable input of providers on the ground, in both hospital and community-based settings, who work daily to care for people experiencing homelessness.

The toolkit consists of a document summarizing our findings from several in-depth interviews with providers on the ground, in both hospital and community-based settings. In addition there are videos and checklists for the providers, social work and discharge planning teams, Nurses, OTs and PTs.

The goal of this toolkit is to provide concrete resources to hospital staff to support the care and discharge planning process for patients who are unstably housed. The toolkit can be used in its entirety or can be divided into multiple sections that can be taken as individual resources for specific disciplines or providers. The pieces of the toolkit can be used to collect valuable information that should be shared during case conferencing/rounding to support individuals who do not have access to stable/permanent housing upon discharge.
