Description: In 2005, Dignity Health, Kaiser Permanente, Sutter Health, and UC Davis Health partnered with WellSpace Health to launch the Interim Care Program. This panel of experts will discuss the program’s successes and challenges around the medical complexity and social health needs of today’s patient population, key learnings, best practices, and opportunities.
