Project H.O.P.E.
Project H.O.P.E. uses a multifaceted approach, including mobile health outreach, street outreach, and primary care clinics, to provide healthcare to individuals who are homeless. These components are intended to fill the gaps in the continuum of services which have been identified through a network of homeless service providers in the city and through Project H.O.P.E.’s 26 years of experience.
Services
Primary Care, Behavioral/Mental Health Care Services, Medication-Assisted Treatment for Opioid Use Disorder, Case Management, Flu Vaccinations, Health/Nutritional Education, HIV/AIDS screening/education, Mobile Medical Services.
Linkages
Project H.O.P.E. partners with several other agencies in Camden City to provide a continuum of care and smooth transition between services. These agencies include, but are not limited to, Camden Area Health Education, The Bridge, Cathedral Kitchen, Joseph’s House, and Volunteers of America – Delaware Valley.
