Outreach Community Health Centers provides directly and/or subcontracts for services for medical, dental, mental health, substance abuse, emergency housing, crisis intervention, prenatal care coordination, case management/ protective payee, outreach, and advocacy. Outreach Community Health Centers also provides HIV/AIDS prevention, testing, and counseling.

Services

Direct services include mental health, case management for persons with severe and persistent mental illness, persons with substance abuse issues, and persons with HIV/AIDS, protective payee assistance, prenatal care coordination, transitional housing for families, substance abuse counseling, HIV/AIDS services and primary care on site and subcontracted at three locations.

Linkages

Outreach Community Health Centers has a presence at 22 locations throughout the Metropolitan area, including three Outreach Community Health Centers-supported clinics.

This model allows service coordination and access to pharmacies, laboratories, radiology, and specialists. Case managers/protective payee workers and medical social workers provide services in 12 sites. Outreach Community Health Centers participates in the City of Milwaukee “Immunization 2000 Program,” and the Wisconsin “Healthy Start” and “Healthy Check” programs. Coordination of services is enhanced through participation in the Milwaukee County Shelter Task Force, which has developed a community-wide Continuum of Care process. Linkages are also with the Milwaukee Healthcare Partnership, National HCH Council, Milwaukee County Behavioral Health Division, Milwaukee County Adult Services Division, Milwaukee County Housing Division, Wisconsin Department of Health and Family Services, and Wisconsin Primary Health Care Association.
