Services, provided at three shelter clinics and through street outreach, include primary care, outreach, women and children’s clinic, dentistry, podiatry, vision care, pharmacy, TB/STD testing, immunizations, mental health/substance abuse counseling, case management, health education, referrals and jail inreach project. Asthma, Behavioral Health Care Services, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Contraceptives (issued w/counseling), Dental Care, Diabetes Testing/Care, Electronic Medical Records, Family Planning, Flu Vaccinations, Gynecology, Hepatitis C screening/treatment, Immunizations, Labs, Medications/Prescription, Mental Health Counseling, Mobile Van/Transportation, Multidisciplinary Health Teams, Optometry Services, Outreach, Pediatrics, Pharmacy, Physical Exams, Pneumococcal vaccine, Primary Health Care, Referrals for Specialty Care, Smoking Cessation, TB screening/education, Treatment of Acute Illness, Treatment of Chronic Illness, Urgent Care
