Franklin Primary Health Center operates the H.E. Savage Memorial Healthcare for the Homeless Program, which provides multidisciplinary services to individuals who are homeless or are at risk of being homeless as defined by Department of Human Services Section 330 (h) Grant Program.

Medications are provided through Franklin’s Pharmacy, Ozanam Pharmacy, and prescription assistance programs

In addition, behavioral health specialists, medical social workers, outreach case managers, and support staff are available to provide comprehensive services. Substance abuse assessment and referral services, as well as mental health services, are provided by licensed and credentialed professionals. Social services also include food, medications, assistance with housing, and additional referrals to community agencies. HCH also provides a dedicated van service.

Linkages

Mental health services are provided on-site and by referral to the Mobile Mental Health Center. Residential substance abuse treatment is provided by referral to the Mobile Rescue Mission, Salvation Army, Wings of Life, and Franklin Primary Health Center’s Substance Abuse Program. We work closely with Housing First Inc. to assist our clients in obtaining housing. Staff also works with Social Security Disability to help clients acquire their benefits.
