Formerly Linn Community Care, Eastern Iowa Health Center is a community health center dedicated to providing quality Family Practice and OB/GYN services in Cedar Rapids, Iowa. The National Committee for Quality Assurance (NCQA) awarded Eastern Iowa Health Center Level 3 Patient-Centered Medical Home (PCMH) Recognition, the highest level of recognition available.

Our standards emphasize partnerships between individual patients and their primary care provider, and when appropriate, the patient’s family. Eastern Iowa Health Center is devoted to the PCMH health care model, and strives to continue providing quality care to maximize health outcomes for our community.
