Covenant House New York – Under 21, Inc.
Covenant House New York (CHNY)/Under 21 provides comprehensive pediatric, primary, and preventative care to homeless adolescents, young adults, and their children. All services are provided regardless of a patient’s ability to pay.
The Health Center is easily accessible, and is located in CHNY’s Crisis Center. We provide a number of services on-site, including pediatric and adult primary care, pre- and post-natal care, STD screening (including HIV rapid-testing), health education, and referrals for specialty medical care, including referrals to services for patients 25 years and older.
Referrals are often written for: substance abuse treatment, radiology, oral health, vision services, x-ray and other specialized care that we currently do not provide on-site. We have a linkage agreement with the Ryan center to preform these services.
Services
The clinic is located in the Crisis Center and is easily accessible to youth experiencing homelessness. It is a fully equipped clinic with on-site laboratories and x-ray facilities. The clinic is staffed by a medical director, pediatrician, physician assistant, pediatric nurse practitioner, and RN.
Comprehensive primary health services, mental health services, social services including eligibility assistance, substance abuse services, and outreach services are provided.
Covenant House operates Community Resource Centers in all five boroughs to provide social services where clients dwell. Adult/Vocational Education, Asthma, Basic Needs, Behavioral Health Care Services, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Diabetes Testing/Care, Electronic Medical Records, Flu Vaccinations, Health/Nutritional Education, HIV/AIDS screening/education, Immunizations, Labs, Medications/Prescription, Mental Health Counseling, Mental Health Services for Youth, Outreach, Pediatrics, Physical Exams, Primary Health Care, Referrals for Specialty Care, Residential, Sexual Abuse Support Services, Smoking Cessation, TB screening/education, Well Child Check-ups
Linkages
We have a number of linkages with other clinics and health centers that service the homeless, including: the William F. Ryan Chelsea-Clinton Community Health Center, Harlem United, Project Renewal, the Institute for Family Health, and the Bedford-Stuyvesant Family Health Center. We have informal collaborations with several local hospitals, to ensure access to emergency and specialized care for our patients.
