CommUnityCare
CommUnityCare operates a multi-site HCH project that provides comprehensive services in Travis County. The project’s various sites, modes of service delivery, and community partnerships target services to three distinct subpopulations: single adults, unaccompanied youth, and women and children who are victims of domestic violence. The project is managed and operated as part of the CommUnityCare system.
Services
The CommUnityCare HCH project serves single adult men and women at its clinic within the Austin Resource Center for the Homeless (ARCH), a low-demand shelter and day resource center in downtown Austin. In addition to primary care, the ARCH clinic provides integrated behavioral health services that includes psychiatry and behavioral health consultant services provided by masters level clinicians. Intensive case management services provides linkages to a wide array of housing, legal, substance abuse treatment, and other services offered by co-located agencies. The CommUnityCare HCH project provides acute care to a smaller number of single adult men, women, and families at the convenient care clinic on William Cannon Blvd. The CommUnityCare HCH project also provides medical care to women and children at SafePlace, a shelter for victims of domestic violence, and to unaccompanied youth at LifeWorks outreach center. Dental care services are provided via the CommUnity Care dental clinics. Vision exams and eyeglasses are provided via the Right to Sight Service located within First United Methodist Church on Lavaca Street, Austin. Pharmacy services are provided through the CommUnity Care’s pharmacy program. CommUnity Care’s longstanding relationship with the local mental health center, Austin Travis County Integral Care, allows for close coordination between HCH staff and the mental health center’s ACCESS program. An ACCESS psychiatrist treats patients 2.5 days per week at the ARCH clinic. Primary Care, Behavioral Health, Case Management, Co-location with the Austin Resource Center for the Homeless (ARCH)

Linkages include Austin Travis County Integral Care, Front Steps (ARCH shelter and day resource center), Caritas’ Re-Entry Program (case management and transitional housing), SafePlace (domestic violence), LifeWorks (youth outreach), People’s Community Clinic, Trinity Center-St. David’s Episcopal Church, Austin-Travis County Health and Human Services Department Communicable Diseases Unit, Austin-Travis County EMS, Brackenridge Hospital, Austin-Travis County Medical Assistance Program, Salvation Army, Austin Recovery, and St. David’s Foundation.
