Community Health Association of Spokane (dba CHAS Health) provides personalized, patient-centered care to homeless individuals through an integrated health care delivery model.

Services

CHAS provides primary medical, dental, nutrition, behavioral health and pharmacy services at five clinic locations in Spokane County. Services include women’s health care, prenatal and obstetrics, maternity support services, health nutrition/education, well child check-ups and immunizations, chronic illness management, HIV/AIDS, outreach, case management, eligibility assistance, transportation, translation, and referrals to community resources as needed.

Linkages

The project maintains linkages with specialty physicians, hospitals, community mental health/substance abuse programs, homeless shelters, social service providers, transitional housing units, employment services, agencies in the private sector, universities, schools, and churches. CHAS is a founding member of the Spokane Homeless Coalition and participating agency in developing Spokane’s Ten Year Plan to End Homelessness.
