Community Action Partnership of Natrona County – 12th Street Clinic
The 12th Street Health Care for the Homeless Clinic (HCH) was established by the Community Action Partnership of Natrona County in 1992 and provides primary health care and ancillary services on-site at Life Steps Campus.
The clinic provides primary medical care, case management, outreach, mental health/substance abuse services, and on-site pharmaceutical services. Our clinic is also a door to more specialized services. Clients become eligible for our services if they: live on the streets, in abandoned building, or in a vehicle; are residents in emergency shelters or transitional housing; are doubled up temporarily with another family; live in substandard housing; or are hospitalized or incarcerated and have no permanent place to live upon release.
Patients remain eligible for services for up to one year after they obtain permanent housing. Our services are not free; we are required to have a sliding fee scale starting at $2.00. Due to the nature of the patients being seen at HCH, a person may come in with an array of complex health care needs, some of which are beyond the capacity of our primary health care provider to manage successfully.
Services
The 12th Street Clinic provides a comprehensive health care program to meet the unique needs of individuals who are homeless.
Outreach and case management are provided and staff works closely with other community agencies to ensure that individuals benefit from available services. Services include primary care, preventive, limited dental and vision care, and medication.
Mental health and substance abuse counseling are provided on-site through a contract with Central Wyoming Counseling Center. Asthma, Basic Needs, Behavioral Health Care Services, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Child Development Services, Contraceptives (issued w/counseling), Dental Care, Diabetes Testing/Care, Electronic Medical Records, Family Planning, Flu Vaccinations, Gynecology, Health/Nutritional Education, Hepatitis C screening/treatment, HIV/AIDS screening/education, Immunizations, Labs, Medications/Prescription, Mental Health Counseling, Mental Health Services for Youth, Mobile Van/Transportation, Optometry Services, Outreach, Pediatrics, Pharmacy, Physical Exams, Pneumococcal vaccine, Primary Health Care, Referrals for Specialty Care, Residential, Services in Supportive Housing, Specialty Care, Substance Abuse Treatment, TB screening/education, Well Child Check-ups, X-ray
Linkages
The 12th Street Clinic has linkages through qualified services organization agreements. These agencies include emergency shelters, transitional housing programs, substance abuse treatment programs, emergency services programs, the local housing authority, Community Health Center of Central Wyoming, Wyoming Medical Center, WIC, vocational rehabilitation, employment service center, schools, Covering Kids Outreach, Department of Family Services, and AIDS programs. The clinic is active in the Casper Area Homeless Collaborative, a regional continuum of care, and the State Homeless Advisory Council.
