The North Broward Hospital District Health Care for the Homeless Program provides primary health care and related services to individuals who are homeless in Broward County at fixed and mobile locations. Among four hospitals and fourteen health center clinics under Broward Health’s purview, the HCH-specific site is located at at 1101 W Broward Blvd.

Primary Health Care Diagnosis and Treatment for Adults and Children, After-Hours On-Call, Behavioral/Mental Health Counseling and Treatment, Preventative Health Screenings, Case Management/Social Services, Chronic Disease Management, Dental/Oral Care, Nutritional Education, HIV/AIDS, Screening Education, Immunizations, Interpretation Services, Lab and Diagnostic Testing, Medical Respite, Mobile Medical Services, Vision Care/Optometry Services, Street Outreach, Pharmacy and Patient Services, Referrals to Specialty Care, STD Testing and Treatment, Referrals for Substance Abuse Treatment, TB Screening/Education, Tobacco Cessation, and Limited Transportation Assistance.

Linkages

Specialty care is provided by referral through both contracted and non-contracted professional services agreements with local providers. These services include oral surgery, dentures, and ophthalmology/optical services. Emergency care, inpatient/outpatient services, and other specialty care services are provided by North Broward Hospital District medical centers and ambulatory locations.
