Also known as Southeast Virginia Health Systems, 48th Street Physicians is a private, nonprofit community health center that provides primary care service for medically indigent person who are homeless in Newport News, Hampton and Suffolk.

The Health Care for the Homeless Program provides direct medical care and case management services. Primary care and prevention are the focus of the medical team, comprised of physicians, nurse practitioners, case managers and outreach workers. Primary care services can be accessed in one of four office locations within the cities of Hampton, Newport News and Suffolk.

Ongoing health care and specialty medical services are provided through referral to participating health centers and doctors. Services include acute and chronic medical care, oral health care, medication assistance, and transportation. Referrals are made for mental health and substance abuse services and for specialty medical services. Case Managers assist with access to enabling services.

Services

The Health Care for the Homeless Program provides direct medical care and case management. Primary acute care and prevention are the focus of the medical team, comprising a physician, nurse practitioner, physician’s assistant, and case managers.

The program provides services to people who are homeless through the use of Hampton University Nursing Center mobile health van and a treatment center. In addition, an evening clinic is held at a local shelter.

Ongoing health care and specialty medical services are provided through referral to participating health centers and doctors. Services include physicals, dental, prescriptions, pregnancy tests, and assistance with applications for food stamps, Social Security disability benefits, etc. Referrals are made for mental health and substance abuse services. Case managers advocate for clients, set up initial appointments, track health records, provide transportation, and monitor transfers.

Linkages

PICH’s Healthcare for the Homeless maintains a collaborative relationship with all local agencies providing services to homeless individuals to include local Departments of Social Services, Health Department, Salvation Army, Probation and Paroleas well as local churches. Additionally, cooperative relationships exist with substance abuse and mental health treatment facilities. Local agencies include Hampton /Newport Community Services Board, Western Tidewater Community
