Ventura County Community Health Center provides primary health and support services at 18 FQHCs throughout Ventura County, including the Health Care for the Homeless Clinic. The VC HCH team includes a medical director, contract physicians, specialists, clinical assistants, and a network of support services.

Services

Medical teams provide services at 18 sites in eight cities. Services include primary and preventive health care and referrals for mental health, substance abuse, social services, entitlement eligibility, TB/STD clinics, and HIV testing. Laboratory and radiology services are provided through Ventura County Medical Center, Ventura County Santa Paula Hospital, and Ventura County Health Care Agency outpatient clinics. Access to program information is provided by all area service providers.
