Valley Health Systems (VHS) coordinates a statewide Health Care for the Homeless program, administered through five organizations located in the cities of Huntington, Charleston, Wheeling, Beckley, and Morgantown.

Services

VHS, in conjunction with the Cabell-Huntington Coalition for the Homeless, provides direct services — via a team model — out of Harmony House and administrative oversight through subcontracts with four other organizations. Harmony House is a multi-purpose center for persons experiencing homelessness. The health care team comprises a nurse practitioner, counselor, medical director, dentist, and social and outreach workers. Services include primary medical care, substance abuse/mental health assessment, counseling and referral, transportation, dental care, showers, laundry, and assistance in acquiring housing, employment, and other social services. Basic Needs, Behavioral Health Care Services, Blood Pressure Monitoring, Case Management/Social Services, Dental Care, Diabetes Testing/Care, Flu Vaccinations, Immunizations, Labs, Mental Health Counseling, Mobile Van/Transportation, Outreach, Physical Exams, Referrals for Specialty Care, Treatment of Acute Illness, Treatment of Chronic Illness

Linkages

The project works closely with Cabell-Huntington Coalition for the Homeless to arrange linkages with other resources and services in Huntington.
