Southern Illinois Healthcare Foundation
Initiated in 2007, Southern Illinois Healthcare Foundation’s (SIHF) Health Care for the Homeless Program provides services to those facing homelessness in St. Clair and Madison Counties,Illinois. The SIHF medical mobile unit, which is equipped with two exam rooms, travels to shelters and food pantries throughout Madison and St. Clair Counties, Illinois.
Services
Primary care services are co-located in a community mental health agency. There is a two exam room mobile medical unit that travels to shelters and other service providers throughout Madison and St. Clair Counties. Additionally, dental services are provided in a dental office immediately across the street from a large shelter in Granite City, Illinois. Behavioral Health Care Services, Blood Pressure Monitoring, Case Management/Social Services, Dental Care, Diabetes Testing/Care, Flu Vaccinations, Health/Nutritional Education, HIV/AIDS screening/education, Immunizations, Labs, Medications/Prescription, Mental Health Counseling, Mobile Van/Transportation, Outreach, Physical Exams, Primary Health Care, Referrals for Specialty Care, Specialty Care, Treatment of Acute Illness, Treatment of Chronic Illness, Urgent Care
Linkages
SIHF has linkages with more than 30 local agencies through partnership with the St. Clair County Homeless Action Council and the Madison County Continuum of Care.
