Solano County Health and Social Services has a Primary Care Support Team of specialized health care professionals who serve homeless individuals at eight access points: Family Health Services clinic locations (five locations in Vacaville, Fairfield, and Vallejo), a mobile dental clinic, and two satellite medical clinics located within homeless support services. The Family Health Services clinics offer primary care, behavioral health screenings, and comprehensive dental care. Evening and weekend clinics occur on a rotating basis.

Services

Physicians, licensed social workers, dentists, case managers, and outreach workers conduct outreach and engagement and provide access to ongoing treatment and services. Mobile services, satellite services, convenient clinic hours, and transportation support the efforts of the team.

Linkages

Linkages have been established with providers of safety net services, including supported housing, food banks, and vocational training.
