At Santa Rosa Community Health, we believe health care is a fundamental human right—everyone should have the opportunity for a full and healthy life. Every day, we open our doors to care for patients in a way that honors them and meets their health care needs.

None of us should ever receive anything less than the best care possible.

Since opening in 1996, Santa Rosa Community Health has developed a range of programs and services to meet the health care needs of our community’s diverse population. Starting with a single primary care health center, we’ve established strong partnerships, expanded our capacity, added care in new locations, and increased cultural acceptance of our services with specific populations. Today, we provide close to 50,000 diverse people who have low-incomes with outstanding medical, dental, and mental health care across twelve sites in Santa Rosa.

Each of our health centers meets general primary care needs and/or the unique needs of special patient populations such as mothers and children, teens, people who are homelessness, transgender, HIV positive and those recovering from drug addiction.

SRCH has several program designed to provide more intensive care for vulnerable high need patients, including intensive outpatient care management, linkages for mentally ill patients to primary care and education programs designed to improve health outcomes, and homeless care transitions.
