Samaritan Health Center (formerly the Samaritan Homeless Clinic) is an ambulatory care department of Five Rivers Health Centers located near the Families and Women Gateway Shelter. All integrated, comprehensive, multidisciplinary services are housed in one site. Off site, the clinic runs an apartment respite care program.

Linkages

The Samaritan Homeless Clinic is active in Shelter Providers Committee and Emergency Housing Coalition, a group of some 28 public, private, and not-for-profit agencies that have a major involvement with persons who are homeless and collaborate to maximize services and reduce duplicate efforts.

The clinic also participates in the Continuum of Care process, Ten Year Plan to End Homelessness, and in several U.S. Department of Housing and Urban Development (HUD) Supportive Housing Programs. Patients graduating from the clinic are transitioned to neighborhood health centers with a sliding scale fee. The Combined Health District provides HIV education/testing, TB care/follow-up, and childhood immunizations. The AIDS Resource Center Ohio provides medical care, case management, and housing.
