For patients experiencing homelessness, health issues are easily exacerbated by the harsh conditions and stress of living without homes. Access to coordinated, comprehensive health care and community services is essential to providing better health outcomes, but navigating community health systems can be especially difficult for homeless populations.
We are pleased to provide this on-demand webinar as a follow-up to Serving Patients Experiencing Homelessness in Hospital Systems. Participants will learn strategies for addressing barriers to care within the community. Learners will receive information about best practices in hospital/community collaborations, engaging hospital staff in the Continuum of Care, and community navigation services for patients experiencing homelessness.
