We know that understanding and addressing the Social Determinants of Health is essential to improving the health and quality of life for health center patients. On a community level, knowing what patient needs are and coordinating across systems is often easier said than done. Using data-driven approaches to identify existing needs, resource gaps, and partner across systems can help health centers coordinate care and connect patients to community resources.
Cross-sector data sharing is a key component of coordination and can take many different forms. This webinar will highlight how one community is working to connect health and housing services with a data-driven, coordinated approach.
