Nasson Health Care is located in Springvale, Maine, within the State’s southernmost county of York. We serve homeless individuals and residents of public housing from throughout the York County, as well as the general population of Sanford-Springvale and its six surrounding towns (Acton, Alfred, Lebanon, North Berwick, Shapleigh and Waterboro). Nasson Health Care is a program of York County Community Action Corporation, whose other programs include Community Outreach, Energy Assistance (including LIHEAP and Weatherization) Head Start and Early Head Start, Housing Services, Transportation, and WIC.

Nasson Health Care provides integrated medical, dental, and behavioral health care, as well as care management and enabling services. We are accepting new patients. All forms of insurance are welcome, and we are happy to explore payment options for those without insurance.

Services

Nasson Health Care offers comprehensive primary care services, including physical exams, Well Child Visits and check-ups, Medicare Wellness Visits, immunizations and vaccines, health education, screenings, referrals to specialty care, and treatment and management of chronic disease, including asthma and diabetes.

Nasson Health Care is a nationally-recognized Patient-Centered Medical Home, meaning that, as a patient, you will work with your primary care team to assure your complete health needs are addressed. If you struggle to manage a chronic disease, we offer behavioral health services that are fully integrated with primary care. We recognize that you cannot be fully well if you are battling with depression or anxiety, or if you struggle with obesity, unmanaged diabetes, COPD, or other illness. We are here to connect you to the resources and services you need, and to provide ongoing support with your self-care.

As a primary care patient, you also have access to our dental program, which offers exams and evaluations, consultations, dental X-rays, cleanings, sealants, fluoride applications, restorative procedures, dental trauma management, referrals, and emergency dental care.

Linkages include York County Shelter Programs, Sanford Housing Authority, Maine Primary Care Association, Counseling Services, Inc., Sweetser Homeless Youth Outreach Services, Goodall Hospital, University of New England Schools of Medicine and Allied Health Professions, Maine Medical Center, Partners for Healthier Communities and York District Public Health Council.
