The Multnomah County Health Department operates the Westside Health Center, a large primary care clinic located in downtown Portland. It also provides services to a satellite clinic located within a local dining hall in Southeast Portland.

In addition, the Health Care for the Homeless Program also operates a smaller primary care clinic, La Clinica de Buena Salud which targets health care services to homeless families in a largely Hispanic Community.

Services

The HCH has strong linkages with the Coalition of Community Clinics, the safety net clinics. We have worked with Corrections Health to take discharges directly from the County Jail and prisons. The clinic also accepts weekly referrals from ten local agencies.

The satellite clinic is located in the St. Francis Dining Hall, which also houses a social worker. Basic Needs, Behavioral Health Care Services, Blood Pressure Monitoring, Case Management/Social Services, Diabetes Testing/Care, Flu Vaccinations, Immunizations, Labs, Medications/Prescription, Mental Health Counseling, Outreach, Pain Management Services, Physical Exams, Primary Health Care, Referrals for Specialty Care, X-ray
