The Healthcare for the Homeless Program at Bergen Ave. provides primary care, integrated behavioral health services, referral for HIV-antibody testing, outpatient substance abuse counseling, and screening services for diabetes, hypertension, and tuberculosis. Referrals are made for gynecological, dental, mammogram, and podiatry services. The programs use a variety of outreach methods and community connections to identify and serve homeless individuals, particularly those with co-occurring substance abuse and behavioral issues. Staff members include physicians, a nurse practitioner, a licensed social worker/behavioral health consultant, counselors, and a van driver.
