HealthFirst Bluegrass, Inc. operates the Homeless Health Clinic at the Hope Center. The primary locations for services are at the main clinic sites, 1640 Bryan Station Rd and 496 Southland Dr. Access to emergency care, specialty care, and inpatient hospital care is facilitated through HealthFirst Bluegrass, Inc and its existing network. In addition to providing primary care, clients can be referred through cooperative arrangements for substance abuse treatment and mental health counseling, personal needs, and other social services.

Services

Health care is provided by HealthFirst Bluegrass, Inc. directly and through subcontracts at the Hope Center. The clinic is open Monday through Friday, and the staff includes a nurse practitioner, medical assistants, and social workers. Services include diagnosis and treatment of medical conditions, lab tests, x-rays, basic medications, and referrals for services not provided on site.

Linkages

The Hope Center provides emergency shelter, food, and clothing to people who are experiencing homelessness. The Hope Center is a multi-location, comprehensive social service agency delivering a variety of programs to help people get off the street and stay off the street.
