Through a mobile health team, Hamilton Community Health Network (HCHN) provides comprehensive primary health care to persons who are homeless in Genesee County at seven clinic sites in missions, shelters, and other multi-service agencies.

Services

An outreach coordinator, mid-level provider, caseworker, and a medical assistant staff the HCHN homeless program. Services include family planning, prescription medication, laboratory testing, preventive dental services, transportation, OB program, TB testing, substance abuse, and mental health referrals.

A Family Independence Agency eligibility worker, stationed at the main clinic, assists clients in applying for benefits and entitlements. In addition to providing medical services, the mobile health team places strong emphasis on providing health education on the non-medical factors that influence homelessness in the community. This approach is an attempt to treat the whole person both physically and mentally as part of the case management process.
