Golden Valley Health Centers Corner of Hope Program exists to increase the health status and health outcomes for individuals and families who are homeless by increasing access to comprehensive primary health care, substance abuse programs, dental services, and mental health services. The program strives to fill gaps in homeless services in the rural county of Stanislaus. The program stresses wellness and health promotion for individuals who are homeless.

Golden Valley Health Centers Corner of Hope Program in Modesto, & our Mobile Outreach Primary Care Unity in Merced, CA exists to increase the health status and health outcomes for individuals and families who are homeless by increasing access to comprehensive primary health care, substance abuse programs, dental services, and mental health services.

Services

Services are provided 5 days a week. Primary care includes episodic, chronic care, well child exams, women’s health, perinatal and GYN care, physical exams, diagnostic services, pharmaceuticals, dental care, vision referrals, health education, some mental health services, and supportive services for substance abuse issues. Patients are offered referrals and case management services. Hospital and after hours care are available through Golden Valley’s on-call physicians. Volunteers are welcome to assist paid staff.

Collaborative links include Community and Housing and Shelter Services, Salvation Army, Modesto City Schools, Department of Mental Health, Department of Social Services, various churches and ministries, Modesto Gospel Mission, Child Health and Disability Prevention Program, and Valley Mountain Region Center.
