Generations Family Health Center, Inc.

Generations Family Health Center receives Health Care to the Homeless (HCH) funding agency wide. In addition, each site uses various resources to provide quality healthcare to homeless individuals. Some examples of the type of resources utilized are as follows: local hospitals’ mobile van, university medical students, emergency shelters, soup kitchens, and faith-based organizations. Educational activities, flu vaccine clinics and blood pressure screenings are some of the ways we are able to provide the much needed quality health care to the homeless population. The success of Generations programs are contingent upon the team approach with various social services agencies. Our mission involves commitment to provide quality, compassionate and professional health care that is affordable, easily accessible and without discrimination to all members of the communities we serve.

24 hour emergency on-call, Asthma treatment, Behavioral/Mental Health Care Services, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Chronic Bronchitis/Emphysema, Contraceptives (issued w/counseling), Dental/oral Care, Diabetes Testing/Care, Documenting Disability Services, Family Planning, Flu Vaccinations, Health/Nutritional Education, HIV/AIDS screening/education, Immunizations, Medication Prescribing, Mental Health Counseling, Mobile Medical Services, OB/GYN, Outreach, Pain Clinic, Pediatrics, Physical Exams, Primary Health Care, Referrals for Specialty Care, Sexual Abuse Support Services, Shelter outreach, SOAR services, Specialty Care, STD Testing, Street Outreach, TB screening/education, Tobacco cessation, Urgent Care, Well Child Check-ups.

Linkages

Generations has linkages with Reliance House, Southeastern Mental Health Authority, Windham Regional Community Council, United Services, Perception Programs, Access Agency, and Thompson Ecumenical Empowerment Group. Linkages with hospitals, shelters, food banks, and soup kitchens are enhanced by Continuum of Care Teams in Norwich, Willimantic, and Danielson. Social Service agencies and State DSS offices provide entitlement assistance.
