Family Health Centers of San Diego
Family Health Centers of San Diego’s (FHCSD) Healthcare for the Homeless Program serves the entire County of San Diego through its network of medical and behavioral health clinics, community-based support services and partnerships with other social service agencies and homeless providers.
Services
FHCSD’s Healthcare for the Homeless Program reduces barriers and increases access to quality health care to those experiencing homelessness by providing primary and emergency medical services. The program also provides dental, vision, mental health, substance use disorder, case management services and housing navigation services.
Many of FHCSD’s 36 clinics are located near, or within, partner transitional housing programs, including organizations that focus on veterans and homeless youth and families. In addition, FHCSD’s three Mobile Medical Units provide health care services at a variety of locations across San Diego County.
As a proud partner of the National Health Care for the Homeless Council, FHCSD shares in their mission to eliminate homelessness by ensuring health care and secure housing for everyone.
Linkages
The HCH Project has well established collaborations and linkages with many social service agencies and homeless service providers, as well as networking and referral support of more than 200 agencies countywide.
