Community Health Center of the Black Hills Outreach Program is a project of the Community Health Center of the Black Hills, Inc. The program’s main office is located at the Cornerstone Rescue Mission with outreach sites at the mission, as well as Working Against Violence Shelter, Salvation Army, Cornerstone Rescue Mission Women and Children’s Shelter, and Cornerstone Rescue Mission-Men’s.

Services

Primary health care services are provided at all sites. The sites are staffed by a medical provider, nurse, and case manager/mental health counselor. Substance abuse counseling is provided in conjunction with City County Alcohol and Drug Center. Additional service needs are met through referral to the main site of Community Health Cen

HCH refers to private and public agencies whenever services aren’t available in the clinic.
