Community Health Care (CHC) HCH program provides comprehensive primary health care to children and adults who are homeless throughout the Quad Cities area. Services are provided at six outreach locations.

Services

The HCH outreach program provides preventive and acute medical visits, medications, outreach, social service coordination, and case management to men, women, and children who are homeless. Referrals are made for substance abuse treatment and mental health services. The HCH program also provides one emergency dental visit, dental referrals, HIV counseling/testing, shelter, and referrals for housing, job employment, food, and clothing. The program is provided at six sites–one on a full-time basis and five visited weekly or bi-weekly.

Linkages

CHC’s outreach program has extensive linkages with shelters, transitional housing programs, local pharmacies, food pantries, eye care providers, and thrift stores. Referrals are made for mental health services and substance abuse/addictions treatment.
