The HCH Program serves San Mateo County, a culturally and geographically diverse community located directly south of San Francisco. Since its inception in 1991, the San Mateo HCH Program has expanded services and sites to serve homeless individuals and families through a comprehensive health care network that aims to reach all the communities in San Mateo County where homeless populations are concentrated.

Services

HCH promotes comprehensive health care and enabling services for homeless individuals and families through an integrated service network designed to engage and create access to care. Front line services are offered at the 11 SMMC clinic sites, the County’s Public Health Mobile Clinic and Behavioral Health and Recovery Services Department (for case management and access to four regional centers). In order to reach more homeless individuals, HCH contracts with community agencies, including the county’s only 330(e) Community Health Center, fixed and mobile dental clinics and the County’s largest homeless shelter/services provider. HCH patients can also access the full-range of specialty care services at the SMMC Main Campus Clinics. 24 hour emergency on-call, Asthma treatment,  Behavioral/Mental Health Care Services, Blood Pressure Monitoring, Cancer Screening, Case Management/Social Services, Chronic Bronchitis/Emphysema, Contraceptives (issued w/counseling), Dental/oral Care, Diabetes Testing/Care, Electronic Medical Records, Family Planning, Flu Vaccinations, Health/Nutritional Education, HIV/AIDS screening/education, Immunizations, Labs, Medication Prescribing, Medication Dispensing, Mental Health Counseling, Mobile Medical Services, OB/GYN, Optometry Services, Outreach, Pain Clinic, Pediatrics, Pharmacy, Physical Exams, Primary Health Care, Referrals for Specialty Care, Specialty Care, STD Testing, Substance Abuse Treatment, TB screening/education, Transportation services, Urgent Care, Well Child Check-ups, X-rays

Linkages

The project has linkages with more than 30 public and private agencies in San Mateo County. The HCH program provides outreach and case management for health and social services, including San Mateo County Health Services Agency’s continuum of health care, mental health, substance abuse, and more.
