Community Action of Laramie County

Crossroads Healthcare Clinic has two service locations. Our main clinic is co-located with the department of health in Laramie County and our mobile clinic provides onsite services at the COMEA shelter, 1504 Stinson Ave., and other areas in the county. We provide quality primary health and behavioral health care for individuals and families who are currently homeless or low income in Laramie County. The clinic is staffed weekdays and Thursday evenings by a mix of clinic staff and volunteer doctors, nurses, and family nurse practitioners.

Services

The Crossroads Healthcare Clinic provides primary health care with voucher referrals and transportation to community providers for dental, mental health, substance abuse, pharmaceuticals, and radiology care. Although the clinic focuses on providing quality primary health care, staff works with various social service agencies to meet the non-medical needs of individuals and families who are homeless. Additionally, the clinic helps both low-income and homeless individuals apply to various pharmaceutical assistance programs. Asthma treatment, Behavioral/Mental Health Care Services, Blood Pressure Monitoring, Case Management/Social Services, Chronic Bronchitis/Emphysema, Diabetes Testing/Care, Electronic Medical Records, Flu Vaccinations, Health/Nutritional Education, Immunizations, Labs, Medication Prescribing, PATH Services, Primary Health Care, Referrals for Specialty Care, SOAR services, Street Outreach, TB screening/education, Tobacco cessation

Other agencies and programs that are closely involved with the Crossroads Healthcare Clinic include Alcohol Receiving Center, OTC, TRP, Safehouse (emergency shelter for women and children who are in an abusive relationship), Comea House (emergency shelter for those who are homeless), Department of Family Services (public assistance and social services), Departments of Vocational Rehabilitation, Developmental Disability Programs, Medicaid and Medicare programs, Head Start, Community Action of Laramie County, AIDS/Ryan White programs, Wyoming Primary Care Association, Wyoming Coalition of the Homeless, State of Wyoming TB Program, City/County Health Department, Veterans Administrating, and other local community organizations, which include ministerial and volunteer agencies.
