The 2019 Revised Adapted Clinical Guidelines: Medication Management for Patients Experiencing Homelessness Diagnosed with Diabetes

November is National Diabetes Awareness Month! In preparation of this special month, the National HCH Council is sponsoring a webinar to introduce our recently revised 2019 Adapted Clinical Guidelines – Recommendations for the Care of Patients Experiencing Homelessness Diagnosed with Diabetes.

The 2019 updated guidelines were adapted from the American Diabetes Association’s (ADA) Standards of Medical Care in Diabetes—2019 and developed to serve as a reference for members of multi-disciplinary care teams who provide health care and support to individuals experiencing homelessness and at risk for or diagnosed with prediabetes or type 2 diabetes.

This one-hour webinar will feature Dr. Lawrence Chang, Pharmacist from Santa Clara Valley Medical Center. Dr. Chang will focus on the Medication Management section of the guidelines, and highlight some new data on medications used for diabetes control. We hope that by releasing the publication and sponsoring the webinar in October, health centers will have time to review the guidelines and use this important resource to enhance diabetes management efforts during National Diabetes Awareness Month.
