PCMH Updates for Safety Net Providers

The next webinar on the patient-centered medical home will include important updates on 2011 standards for the safety net community as well as a review of past resources and guidance for those new to the process. Topics in the webinar will also include the funding application to cover recognition fees for HCH grantees and examples of successful implementations overcoming common challenges in safety net.
A portion of this webinar will also be devoted to Q&A, and the technical assistance team at the National Health Care for the Homeless Council will be ready to address concerns and schedule follow-ups, as indicated.
