Community Health Worker Guide

This webinar will orient participants to the new guide on integrating community health workers into an health care for the homeless project, with a focus on outreach and enrollment. Two HCH projects’ community health workers will be featured—one whose focus is on navigation and enrollment and the other whose focus is on outreach and education. Learn how community health workers can help your project achieve your outreach and enrollment goals, especially as Medicaid expands to cover more patients served in HCH settings.
