Clinicians’ Coffee Chat: Using Telehealth to Manage Diabetes

In this first of a four-part “Clinicians’ Coffee Chat” series on diabetes, participants will hear from organizations using telehealth for diabetes care and education. This interactive conversation will foster peer learning and support across the HCH community. Please bring your questions and experiences to share in the discussion.

Three additional diabetes coffee chats focused on community health workers, co-morbidities, and nutrition will take place in the winter and spring of 2020.
