Our free online course — Facilitating Connections to Housing and Supporting Housing-Related Needs — is built to strengthen connections between health programs and community partners that specialize in housing navigation, case management, and more.
This course has four learning modules that dive into strategies for health centers and medical respite care programs to increase their capacity for meeting the housing-related needs of their clients.
Module 1: Homelessness Status, Needs, and Housing Resources
Module 2: Identifying and Assessing Housing Support Needs
Module 3: Building Program Capacity to Meet Housing Needs
Module 4: Implementing Health & Housing Partnerships
Throughout the course, learners will review real-world examples from different community programs successfully doing this work, through both internal staff activities and through strong community partnerships.
