Serving Patients Experiencing Homelessness in Hospital Systems

Patients experiencing homelessness have complex needs that are difficult to meet in fragmented systems. Providing coordinated care helps to better manage conditions, reduce emergency department visits, and increase patient outcomes. Hospitals systems are integral partners in serving this vulnerable population.

This on-demand webinar explains the history of the Health Care for the Homeless (HCH) Program and how hospital systems have participated in the HCH model of care. Speakers also share resources and discuss models and strategies to build hospital/HCH relationships.
