Michigan is using a state plan amendment instead of an 1115 waiver. As of September 1, 2024, Medicaid enrolled adults who are experiencing homelessness and being discharged from an inpatient hospital stay, are now eligible to receive Targeted Case Management – Recuperative Care as healthcare benefit. As a transitional program, those who are too ill or frail to return to their living environment, but who are not eligible to continue hospital level care, skilled nursing care, or other inpatient Medicaid services, can utilize up to 90 days of Recuperative Care per hospital stay.
Note: The admission criteria for recuperative care require an individual must have “an acute medical condition that can be addressed in less than 90 days” and must “be medically stable”. In practice providers have reported denials from Managed Care Plans across the state due to the specificity of the criteria (e.g., a chronic medical conditions such as diabetes cannot be cured within 90 days, but the patient was hospitalized due to unchecked blood sugar levels, meaning a lack of medical stability and therefore exclusion from eligibility for recuperative care).
