The goal of this course is to strengthen health program connections to partners in the community that specialize in housing navigation, case management, and more. Included learning modules dive into strategies for health centers and medical respite care programs to increase their capacity for meeting the housing-related needs of their consumers. Throughout the course, learners will review real-world examples from different community programs successfully doing this work, through both internal staff activities and through strong community partnerships.
