Compendium of Housing Resources for Health Care Providers

Introduction:

This resource compendium is designed to support health centers and medical respite care (MRC) programs in better identifying and addressing the housing needs of their consumers.[1]

Below is a list of resources including tools, white papers, briefs, and activities, all focused on two key strategies: improving how housing status and needs are screened and expanding capacity to meet those needs through strong community partnerships. This compendium is broken down into the following sections: Introduction to Health & Housing Connections, Identifying Housing-Related Needs and Resources, Building Capacity to Support Connections to Housing, and Implementing Health and Housing Partnerships.
