
Health centers serving homeless individuals experience unique challenges in meeting clinical quality measures due to the complex needs of the patients they serve.
This community of practice is intended to provide an opportunity to share quality improvement practices with peers and use the expertise in the group to get feedback on current initiatives and generate ideas. Participants will meet for four 60-minute sessions (see dates below).Â
The goal of this community of practice is to be highly interactive, with the expectation that participants will share their experience – both challenges and innovations – to encourage peer exchanges in each session. We ask that you identify one quality improvement process, project, or challenge that you would like feedback on to share with the group throughout this process.
It is our hope that most organizations represented will have the opportunity to share their work. NHCHC will provide office hours between sessions for those who would like support as they prepare to share their work with the group.
Eligibility
Applicants must be representatives of a Primary Care Association, Health Center Program grantee, or potential Health Center grantee.* Each organization may have up to three participating representatives.
*Additional organizations may be approved as space allows.
Fees
No cost. Free.
Commitment
Accepted applicants are asked to attend a total of four, 60-minute, sessions between March and April 2026.
Expected Outcomes for Participants
1) Identify priority conditions of people served by your organization.
2) Assess strengths and gaps of quality improvement process.
3) Utilize feedback from peers to strengthen existing quality improvement processes.

- Session 2 (Wednesday, March 25, 3pm – 4pm ET/12pm – 1pm PT)
- Session 3 (Wednesday, April 8, 3pm – 4pm Eastern/12pm – 1pm PT)
- Session 4 (Wednesday, April 22, 3 – 4pm Eastern/12pm – 1pm PT)
