Peak Vista Community Health Centers
Peak Vista Community Health Centers, Inc. (CHCI) provides primary health care to individuals who are homeless. Services are provided at the Homeless Health Center.
The homeless population in the Pikes Peak region can find exceptional medical, dental and behavioral health services at Peak Vista’s Homeless Health Center. The center is staffed by committed, award-winning providers who treat patients without regard to their residence.
Services include diagnosis and treatment of sudden, minor illnesses and injuries, on-going primary care for prevention and chronic health conditions, women’s health check-ups (including pap smears, mammograms and birth control), minor surgeries, dental services and health education. Should you require specialty care, we are happy to refer you to other qualified community providers and social resources.
Services
A mid-level health professional provides services weekdays at the Homeless Health Clinic located five blocks from the shelter. In addition, a physician provides part-time on-site services and a female clinician provides a women’s health clinic. Staff provides outreach services at the local shelters and homeless camps. To ensure continuity and quality of care, the provider often takes his medical backpack and searches the streets and camps to give patients their test results and treatment plans.
A medical van provides a variety of services to individuals living on the streets. Emergency dental services and substance abuse counseling are also provided on-site. Asthma, Behavioral Health Care Services, Blood Pressure Monitoring, Cancer Screening, Chronic Bronchitis/Emphysema, Dental Care, Diabetes Testing/Care, Electronic Medical Records, Gynecology, Immunizations, Labs, Medications/Prescription, Mental Health Counseling, Mobile Van/Transportation, Multidisciplinary Health Teams, Outreach, Pain Management Services, Pediatrics, Pharmacy, Physical Exams, Pneumococcal vaccine, Primary Health Care, Referrals for Specialty Care, Smoking Cessation, Specialty Care, Substance Abuse Treatment, Treatment of Chronic Illness, Urgent Care, X-ray
Linkages
Substance abuse treatment and counseling are provided through arrangement with the health department’s McMaster’s Clinic. Inpatient detoxification and treatment are arranged by referral. Patients with dental needs beyond the scope of the Homeless Health Center are referred to Peak Vista CHC’s main dental clinic. The Homeless Health Center also has linkages with Marion House and Soup Kitchen social outreach team, Catholic Charities, Urban Peak, El Paso County Department of Health and Environment.
