Formerly Fox Cities Community Health Center, Partnership Community Health Center provides direct medical dental and mental health services to homeless individuals and families in Calumet, Outagamie and Winnebago counties.Services are provided onsite at the Emergency Shelter of the Fox Valley Region.

Services

FCCHC provides on site care to residents of the Emergency Shelter of the Fox Valley region and numerous temporary housing facilities thorouout the area.

A nurse practitioner, substance abuse counselor and the outreach team at the shlter work collaboratively to provide services at the shelter and the main health center site. Transportation and language support are providedto individuals at both the shelter clinic and Health Center’s main location.

Linkages

FCCHC has collaborative partnerships with the area social services organizations as well as the two health systems Affinity and Thedacare. Both organizations provide laboratory and radiology support. Additionally, FCCHC works closely with the five coutny and city public health departments, Well Woman of WI, The Tri-County Community, Goodwill Industries of North Central WI, and Valley Transit.
